The Day Camp Medical is required for all campers. In order to give our camp directors time to review each and every one of our campers medical information, we ask that all medicals are completed and returned by June 1, 2019.  Our form can be downloaded below; however, we gladly accept generic forms from the doctor if the following information is included:

  • Child’s Health History
  • Statement of Health Status
  • Immunization History
  • Must be signed by child’s physician

Unfortunately, due to state licencing guidelines, children cannot attend camp if their Day Camp Medical is missing or incomplete

Consent for Medication

Medication Consent

While at camp, will your child require any of the following?

  • Over the Counter Medication
  • Prescription Medication
  • Asthma Inhaler
  • Epi-pen

If so, the State of New Hampshire requires that we obtain a signed Medication consent.

Medication Consent must be signed by the child’s physician for all epi pens and inhalers

Guide to compeleting the Consent for Medication

  • OTC Medication:
    Parents please complete Section One fully.  Please do not leave any medication with your child. Medication must be in original container with child’s name clearly marked on it and given to the Camp staff at sign in.
  • Prescription Medication:
    Parents please complete Section One fully.  Please do not leave any medication with your child. Medication must be in original container with child’s name clearly marked on it and given to the Camp staff at sign in.
  • Inhalers:
    Parents please complete Section One and Section Two.  Physician must complete Section Three.
    *You may choose to allow your child to carry their own inhaler.
  • Epi-pens:
    Parents please complete Section One and Section Two.  Physician must complete Section Three.
    *You may choose to allow your child to carry their own epi-pen. If you choose this option, please list any adverse reactions that may occur to another camper, for whom the epinephrine/epi-pen is not prescribed, should the camper receive a dose of the medication.

Fare Form (Food Allergy and Anaphylaxis Emergency Care Plan) is required for all children with Epi Pens or Severe Allergies.
